Description:

RESPONSIBILITIES:

This full-time position works the 3 PM - 11PM/ 11pm-7am shift, 6 days on, 2 days off.


Responsibilities include:

• Perform certified nurses aide duties as assigned
• Ensure that all needs (medical, health, safety, social, etc.) of clients are met.
• Provide quality care by teaching and assisting clients with Activities of Daily Living (ADL), such as cooking, grooming, personal hygiene, money management, housekeeping, and medication administration. These functions may be performed at home, day program, in the community, etc.
• Assist clients in developing appropriate social skills such as communicating with others, participating in activities, and practicing good manners.
• Assist clients to effectively deal with matters such as personal problems, personal values, job responsibilities, etc.
• Provide supervision and guidance as outlined in clients' Individualized Service Plans (ISP).
• Administer medications to clients upon successful completion of medication training and annual medication practicums thereafter.
• Participate in trainings designed to develop the knowledge, skills, abilities, and attitudes necessary to assist clients to live and function as independently as possible in the community.
• Maintain certification in first aid and CPR and perform as necessary.
• Assist in the development and implementation of clients' ISPs.
• Assist in developing, implementing, and following clients' individual Social/Emotional/ Environmental Plans, Behavior Plans, and Restrictive Plans.
• Coordinate and handle day-to-day activities of the assigned Community Home.
 Schedule social and recreational activities.
Transport or assist with the transportation of clients to various appointments, community activities, day program, etc. as a group or on a one-to-one basis.
• Attend day program with clients, if required. Report to and follow directives from site Supervisor/Director when working in Lifesteps' day program.
• Assist in maintaining the Community Home in good condition and in accordance with applicable fire and safety rules and regulations.
• Assist clients in managing their individual funds within their budgetary guidelines.
• Document daily all necessary information, including, but not limited to, "happenings" during the work shift, client progress/incidents, medication administration, etc., to ensure smooth communication between shifts and compliance with Agency, legal, regulatory, licensing, and any other requirements.
• Drive/operate various Agency vehicles as required.
• Work shifts as assigned, as well as needed overtime.
• Comply with confidentiality policies and procedures, including, but not limited to the Health Insurance Portability and Accountability Act (HIPAA).

REQUIREMENTS:

• Current Certified Nurse Aide (CNA) Certificate; prefer a minimum of six months of experience as a Residential Aide in Lifesteps' Community Homes Program or other appropriate residential program
• Be at least 18 years of age
• Possess a valid driver's license which legally permits operating a motor vehicle in Pennsylvania.
• Successfully obtain Medication Administration Certification within three months of hire and obtain certification in First Aid/CPR within six months of hire. Prefer current certification in Medication Administration and First Aid/CPR upon hire.
• Drive a vehicle without accidents or violations.
• Lift, carry, push, and/or pull up to 200 pounds with assistance.
• Lift, carry, push, and/or pull up to 50 pounds without assistance.
• Respond calmly in emergency situations.
• Tolerate a high volume of noise from multiple sources (talking, crying, yelling, etc.).
• Work with aggressive, violent, and diverse behaviors.
