Description:

RESPONSIBILITIES:

This is a substitute position that provides services to Lifesteps clients throughout Allegheny, Armstrong, Beaver, Butler, Indiana, Washington and Westmoreland counties as needed but not to exceed 19 1/2 hours per week.
Responsibilities include:
 Perform initial and ongoing assessment of clients ability to perform tasks in living and working environments and write evaluation reports.
 Write individual treatment and activity plans.
 Review treatment plans at least semiannually and revise as needed.
 Write therapy progress reports in accordance with the recommended program schedule.
 Write daily treatment logs of therapy procedures and successes.
 Provide carry-over activities to family and staff and consult as necessary.
 Provide training and inservice as needed for parents and staff.
 Provide treatment based on goals and objectives and monitor the progress.
 Consult with other therapeutic personnel involved with the client.
 Attend at least 24 hours of continuing education each year.
 Complete monthly summary of flow sheets for B.E.S.T. programs related to occupational therapy.
 Refer clients for additional evaluation when appropriate.
 Serve as a member of appropriate multi-disciplinary teams. Actively participate in team meetings and IEP conferences.
 Keep a copy and submit client attendance records to billing.
 Maintain up-to-date knowledge of appropriate tests and materials.
• Provide inservice programs to organizations on issues related to occupational disabilities and other areas of expertise.

REQUIREMENTS:

Master's Degree in Occupational Therapy

PA License as an Occupational Therapist

Prefer experience with children from birth through age 5.
